My Action Planner is implemented based on a proven methodology that helps people with chronic conditions manage their own health. Every week, simply describe your plan with My Action Planner and update your progress daily. At the end of the week, self-evaluate your progress toward your goal and that’s it. For your convenience, My Action Planner also tallies up your self evaluations and gives you graphic reports on how well you have done on different types of activities over time. It is like your mobile personal coach.
