Pediatric to Adult Care

The Pediatric to Adult Care Transition (PACT) mobile application is designed to assist youth who are making the transition from pediatric care to adult care. This app helps ensure that they with know the necessary knowledge and information to continue to receive the care they need. This app was made possible by the efforts of Kimberly Campbell, RN, and Olcay Y.
