About Procedure Log

View demo videos on the app's website at http://goo.gl/ZRC8gt

This app was created for physicians, residents, medical students, and other students in the healthcare fields to keep track of and document procedures performed. The app is highly customizable and useful in all medical specialties.

Entries are securely kept in your iOS device and are not sent to any outside server, website or database. Reports are created and saved as a password protected PDF file (encrypted using AES-128) and a CSV (comma-separated values) file that is compressed to a ZIP file (which allows password protection and is encrypted using AES-256) to comply with HIPAA security requirements. The reports can then be emailed, accessed via iTunes and the Files app, sent using AirDrop, or copied to your iCloud Drive or DropBox. CSV files can be imported into a spreadsheet program.

One should not rely on this app alone to keep track of their procedures and is advised to frequently backup the files created. Information on backing up and restoring your data is included in the app's Help/Instructions view.

FEATURES
• Keep a log of the procedures you have performed, assisted in, were 2nd assist in, observed, supervised, and/or interpreted for documentation purposes.
• Document the patient's name, ID number, age, date performed, location, supervisor, diagnosis, complications and additional notes with the procedures performed. Also, one can take a photo with your device's camera or import a photo from your photos library from within the app, and include the photo in a record.
• Option to select one or multiple diagnoses from a list of either custom diagnoses or ICD-10 diagnoses for any given procedure.
• Select multiple procedures performed on a single patient.
• Sort by date (ascending or descending), patient name, procedure name, and medical specialty.
• Capture supervisor's signature to attest that you performed or assisted in the procedure(s) and include the signature images in your reports. Either obtain the supervisor's signature attestation at the time you create the log entry, or save the entry without the signature and obtain the signature attestation at a later time.
• Detailed help/instructions section.

CUSTOMIZABLE
• List of procedures, favorite diagnoses, and complications are easily customizable (you can add procedures/diagnoses/complications to or delete them from the lists). (ICD-10 diagnoses cannot be edited or deleted.)
• Locations and Supervisor's names are saved and can be selected from a picker so you don't have to type them in again when adding another log entry.
• Add up to four custom fields in addition to the default fields listed above in Features.

REPORTS
• Create reports in password protected PDF format and as a CSV file that is password protected in a ZIP format, as well as a password protected backup of your data, that can then be emailed, sent via AirDrop, sent to your iCloud Drive or Dropbox account, or downloaded using iTunes when your iOS device is connected to your computer.
• Reports can be created for a specific date range, and one can specify the primary, secondary and tertiary sort order of the PDF report.

BACKUP
• In addition to backing up to iCloud and/or iTunes, if you have a DropBox account, you can copy the password protected backup file and reports to DropBox from within the Procedure Log app.
• You can also use the Files app to transfer files to your iCloud Drive, Dropbox account, or, if you have a Mac computer, use AirDrop to transfer it to your Mac computer.
• You can also email the password protected backup file to yourself.

SECURITY
• App is password protected and all data is encrypted when you log off the app or the app is no longer the active app.
• PDF and CSV reports are encrypted and password protected.
• Retrieve your username and password if you have forgotten them; you must first answer several security questions in the "Create User" or the "Edit User" views.

• Minor user interface fixes.
4.2.2
Mar 17, 2023
● In the Create Reports view, the option to include location in the PDF reports was added (it will be off by default) and the option to include the signature attestation was moved to the bottom. (Previously, location was included automatically unless the options to include the patient name and/or patient ID were turned off. Now, if patient name and ID are off, the location can still be included in the PDF report.)

• Add up to 4 custom fields to the Add to Log view (create or edit these custom fields from the Settings view or Add to Log view).
• Reports:
- Any information in the custom fields will be shown within the Notes area in the PDF reports (so will not be shown if "Include Notes" is turned off).
- If "Include Patient Name" and "Include Patient ID" are turned off, then the Location will also not be included in the PDF report.
• After this app version is first run, the Photos folder (if present) will remain unzipped; instead, the folders inside the Photos folder containing the photos associated with each of the logged records will be zipped/encrypted until needed.
• Updated the code that encrypts and zips the files.
• Updated the Privacy Policy and Help/Instructions.
• To add multiple procedures to the app's list of procedures, one can use the app itself or one can use their computer and fill out the form in the app's website at http://rmk-iosapps.com/procedure-log/create-procedures-list.html. A file containing the procedures and the specialty to which they belong will be created and downloaded to your computer, then the file must be imported into the app and can be shared with others if desired.

3.8
Feb 4, 2019
• Added ability to change speed of transition animations in Settings view.
• Added ability to delete entire Specialty when editing Procedures and entire Category when editing Complications.
- Go to Settings > Edit Procedures, then tap the Edit button.
- Go to Settings > Edit Complications, then tap the Edit button.
• Added ability to import additional lists of procedures for various specialties. Lists available with this update are:
- Bedside Ultrasound,
- Emergency Medicine,
- Emergency Medical Services [EMS], and
- Interventional Radiology.
Go to Settings > Edit Procedures, tap the import button in the middle of the toolbar at the bottom, then select the specialty whose procedures you want to import. You'll be taken to the list of procedures for that specialty that you can deselect procedures you don't want imported, then tap on "Import" at the top. It will check whether you already have any of the procedures in your list for that specialty to prevent duplicates.
• When adding a new procedure or complication after tapping "Add Another," the Specialty or Category will be set to the last one added (but can still be changed).
• When adding a new record to your log, the Specialty can now be selected in the Add to Log view, then when you go to the Select Procedure view, that Specialty will already be at the top (though one can still scroll to a different specialty). The selected specialty will become the default.
• Minor fixes and other minor improvements.
3.7.2
Jan 16, 2019
• Fixed some issues that potentially could cause a crash or corruption of the database.
3.7.1
Jan 13, 2019
• Fixed issues that occurred in iOS versions less than 11.0 where the Reports icon would not display in the main Procedure Log view and a crash would occur when trying to view the Help/Instructions view.
3.7
Jan 9, 2019
• Items stored in the Keychain (username, password, your full name, email address, institution name, and answers to security questions, etc.) are now synced to iCloud Keychain, so now there should be no need to create a new user when getting a new phone and restoring from an iCloud backup.
• If you add photos to a record, you can now select frequently used titles for photos from a list. Add to this list in Settings > Edit Photo Titles.
• If the app is moved to the background, one will now be logged out of the app after 60 seconds, not immediately, unless the Logout button is tapped.
• The app will detect if a phone call is received and if you connect to the call while the app is active; one will not be logged out of the app during the call; once the call is ended, you will have 60 seconds to return to the app before being logged out of the app.
• Fixed issue that caused the Photos and/or Signature Images to be deleted if the iPhone auto locked while the app was open.
3.6
Sep 29, 2018
• Added ability to scan barcodes for patient demographic information (name, ID, and age, which is calculated from date of birth). Tap the barcode icon at the top right of the Add to Log view to access the scanner. Note: this scanner may not be able to read some medical barcodes due to certain security features encoded in them.
• ICD-10-CM codes updated for FY 2019.
• One can now change the procedure/specialty of a record after it has been created if there is no associated signature attestation for that record.
• Added default location/facilities: one can now select a location as a default location that will be added automatically to the Add to Log view when creating a new record. Go to Settings > Edit Location/Facilities, then tap the location/facility you want as the default; it will be marked by a checkmark.
• Fixed an issue in the Add to Log view that caused the patient's name, ID, age, etc. from being recorded if a Diagnosis, Complication, or Photo was also added to the record at the time the record was created.
• After entering a patient's name when adding a new entry, the app will now check if a previous patient with the same name exists only if the last name is longer than 3 characters (avoids the alert message if one only uses patient's initials).
• Complication comments can now be added without first having to select from the complications list.
3.5.2
Sep 20, 2018
• Fixed an issue that could cause a crash in some devices if records were sorted by Specialty in the main view. (I was unable to reproduce this, but hopefully was able to fix it.)
3.5.1
Sep 3, 2018
- Fixed an issue that caused a crash when trying to create reports if records were created in more than one region that has different time formats.
3.5
Aug 31, 2018
• App now creates an automatic backup of the database on logout, and one can restore from that backup by going to Settings > Restore Database.
• Now one can use the Files app to view the files/documents in the Procedure Log app and use it to transfer files to and from your iCloud Drive, Dropbox account, or Mac computer using Airdrop.
• Updated the Settings and Backup & Restore sections of Info > Help/Instructions to include a discussion of the new automatic database backup and ability to restore the database from within the app and the use of the Files app.
• Fixed an issue that caused a crash if the manner that records are sorted was changed when no records were entered yet.
• Fixed an issue that potentially could cause a crash when loading the logged procedures.
• Other bug fixes.
3.4.2

• The app will now wait for all photos and signature images (if any are present) to be decrypted/unzipped before logging in to avoid the potential of some images not loading or backup file not including all images.
• Slight speed improvement in creating Backup.zip file by concurrently encrypting/zipping the Photos and Signature Images folders (when present).
3.4.1
Aug 15, 2018
• Fixed an issue where, if one got a new phone, creating a new user would not allow one to enter the same password as before if that password did not meet the new password requirements.
• Other bug fixes.
3.4
Aug 9, 2018
● To improve security of the app and reports, the app password and report password must now be at least 8 and up to 64 characters in length and contain at least:
• 1 lowercase letter,
• 1 uppercase letter,
• 1 number, and
• 1 special character: !@#$%*?&
(special character may also include a space, but not as the first or last character)
● Zip files are now encrypted using AES-256 (a much more secure encryption method). One will need a third-party Zip utility to unzip the files on your computer (free downloads are available for different operating systems):
• for Mac (macOS X): Keka (https://www.keka.io/en/)
• for Windows and Linux: 7-Zip (https://www.7-zip.org).
● Added option to confirm reports password prior to creating reports in Reports Settings area of Create Reports view (option is on by default but may be turned off).
● Updated the Create Reports section of the Help/Instructions page in Info section and added two sections: Backup & Recover, and Printing PDF Reports
● Minor bug fixes and improvements.
