Biofourmis Care™ is a software solution designed to enable site agnostic care by leveraging software and data science to aid clinical care teams in monitoring and managing adult patients. Biofourmis Care™ supports several clinical conditions (e.g., heart failure, hypertension, diabetes) and spans the entire continuum of care acuity including acute, post-acute, and chronic care.
The Biofourmis Care ™ solution also provides a mobile application for healthcare providers to remotely monitor and coordinate care for patients which includes:

Patient communication tools (video, chat)
Ability to view and respond to notifications
