CRITICAL UPDATE: fixed a bug in calculating body surface area for lower GI staging.

"Consider Diff. Diagnosis" has been changed to "Document Additional Cause". This makes downstaging lower GI or liver easier to follow. The downstaging is also separate for the lower GI and liver, so one organ can be downstaged and the other remain as is. Furthermore, we display whether or not an additional cause has been documented under Lower GI and under Liver in the Organ Stage report.
