About Engage AL Transition

This app is for high-school aged students with disabilities to assist with working on transition planning and to identify transition goals as part of their Individualized Education Program (IEP). After completing a brief 10 minute survey, the app produces an individualized transition plan based on your responses. A transition plan is your road map for transitioning to adult life after high school and an important part of your IEP document. Upon completion, you can print or email a copy of your transition plan to yourself or others.

A transition plan helps you move into adult life after high school. Your school must develop a written transition plan as part of your Individualized Education Program (IEP). Having an effective transition plan helps you get the right services and supports so you can work towards what you want after high school. A transition plan can help you prepare to continue your education, train for a job, and live independently.

This app is intended to serve as a guide to Alabama students, family members, and educators during the transition planning process. It is based on the requirements of the Individuals with Disabilities Education Act IDEA - https://sites.ed.gov/idea/. As an Alabama student with a disability who may be turning 16 years or older, (or entering the 9th grade)or a family member of a transition age student, this app can help prepare for the Individualized Education Program (IEP) meeting where transition planning will be discussed and the transition plan will be created or updated. Individuals residing outside of Alabama may use this free app, but the information needed for transition plans in other states may be different.

This app was originally designed by the Disability Rights Center of Kansas, a non-profit organization and adapted by the Alabama State Department of Education, Alabama Department of Rehabilitation Services, Alabama, Alabama Disabilities Advocacy Program and Alabama Council on Developmental Disabilities to meet transition planning needs in Alabama.
