About MyTransition App

The MyTransition App is a tool to help youth feel ready to transition to adult health care system. Transition is a time of change with some new learning, but the MyTransition App has gathered tools to help youth to be organized and confident in this transition. The MyTransition App includes:
- TRANSITION-Q: a quiz that figures out ways to help make your transition to adult care as smooth as possible;
- Calendar: to help you keep track of appointments and prescriptions;
- MyHealth 3-Sentence Summary: a way to summarize your health information to help you speak with doctors, nurses or therapists;
- MyHealth Care Team: a place to enter contact information for your health care providers;
- Goals: allows you to select items from the TRANSITION-Q to set as goals and links with tips on how to improve in these areas;
- Tips: provides helpful advice on each of the 14 TRANSITION-Q items to help you prepare for transition to adult care.
Each of these tools can help you in different ways!

- The calendar section has been added to allow youth to keep track of healthcare appointments and prescription refills.
- Tip sheets for each of the Transition-Q items were developed by McMaster Children’s Hospital and have been integrated into the MyTransition App. These tips comprise a list of “Things to consider” to be successful at achieving a goal and are linked directly to the TRANSITION-Q items- The new goals page allows youth to select up to three Transition-Q items that were scored either sometimes or never. These goals are then populated with tips from the tip section to provide concrete ideas on things the youth could do to improve their health care self-management.
- In our previous version of the MyTransition App, there was no record of TRANSITION-Q scores. By including the TRANSITION-Q history, youth are able to see whether they have improved overall on the TRANSITION-Q and which items may have improved.
